The aim of this paper is to evaluate the safety probabilities of zero-power heavy water reactor (HWZPR) core damage frequency during abnormal operating conditions. Probabilistic Safety Assessment (PSA) is a strong method to assess accidents and investigating the quality of reactor risk. In this method, the operational risks of the reactor core are expressed in different numerical values related to different levels of the accident, including the frequency of damage to the reactor core or environmental risk. In this study, SAPHIRE computer calculations code were used to evaluate PSA first level. Accidents such as loss of coolant in the reactor core and human error were identified as the most important factors of damage to the reactor core. The results showed that the contribution of human error in the core damage of HWZPR reactor is very important hence, the highest frequency of total core damage of the reactor is about 1.44E-04.
